Your First Visit
Please arrive about 10 minutes early so that you have enough time to fill out a New Patient form.
- Initial Appointment
Part of your initial appointment will involve a consultation explaining your diagnosis and treatment options.
You will have an opportunity to ask any questions you may have regarding your diagnosis and proposed treatment plan. Your involvement and opinions about your choice of treatment are essential to ensure an effective, successful treatment.
We will discuss your expectations and concerns during the initial consultation. Details of procedures, recovery time and more will also be discussed.
Often, surgery can be performed the same day as the consultation. However, if you have a complex medical history or require a customized treatment plan, we will need to do an evaluation and schedule a second appointment to provide treatment on another date. - What We Need From You
Please help us by providing the following information at your consultation:
- The referral form your dentist will give you, with details of treatment(s) required
- Any recent, relevant x-rays from your dentist that we can use in the treatment process (our office can take additional x-rays when required)
- A list of your current medications, if any (e.g. heart medications, aspirin, anticoagulant therapy, etc.)
Please notify the office prior to surgery if you have a medical condition that may be of concern (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication.

Pre-Operative Consultation & Physical Examination
A pre-operative consultation and physical examination are mandatory for all patients undergoing IV sedation or general anesthesia. You will also need an adult present in the office during surgery. This person must be able to transport you home following the procedure.
- Minors Under 18
All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.
- X-Rays
If your dentist has taken x-rays of your mouth, you can ask them to email or post them to us or bring them with you on your first visit. If these x-rays are relatively recent and show the information and structures needed to perform the treatment, we will use it. Otherwise, a new x-ray can be taken at our clinic.
